Information for New Patients

You can also fill out the forms online.

During your visit, we may dilate your eyes with drops to aid in the visualization of the back of the eye.  These drops last about 4-6 hours, so you will want to wear sunglasses afterwards.  Most patients can drive while dilated, but some prefer to have someone drive them home afterwards.

Insurance carriers are subject to change and this list is not a complete list of all carriers.  Smaller carriers are often underwritten by one of the companies listed above -- check your insurance card for details. 

Some patients do not realize that their medical insurance often covers the majority of the cost of an eye exam.  While your medical insurance may not cover "routine vision", it will cover visits for a medical reason (i.e. blurry vision, cataracts, itching, pink eye, diabetes, etc.).  Vision insurance takes over where medical insurance ends by covering the parts not traditionally covered by insurance and the occasional pair of glasses or contact lenses.  For most patients, vision insurance is not worth the premiums.

North Texas EyeMD does not participate in any vision plans, but some vision plans will reimburse you for seeing an out-of-network provider.
